Job summary

DaVita IKC is seeking a Registered Nurse Case Manager in Sacramento to coordinate care for ESRD and CKD patients. The role emphasizes collaboration with physicians, care teams, and families to improve outcomes and reduce fragmentation of care.

The position requires at least five years of clinical nursing experience, with three years in renal nursing preferred, and an active RN license. CPR certification is mandatory, and Capella experience within 90 days is expected.

Qualifications

• Active RN license in state of practice.
• Minimum five years of clinical nursing experience.
• Three years renal nursing preferred.
• Case management experience required; alternatively 3+ years coordinating care.
• CNN or CCM preferred.
• Current CPR certification.
• Home office with reliable internet connectivity.

Responsibilities

• Coordinate care between patient, family, physician, and care teams.
• Collaborate with DaVita IKC and nephrology partners to optimize patient care.
• Support ESRD/CKD patients through complex healthcare transitions and coordinated care.
• Utilize Capella and other DaVita clinical software to manage patient needs.
