Job summary

DaVita Integrated Kidney Care (DaVita IKC) seeks an experienced RN Case Manager for a hybrid role covering York, Harrisburg, and West Philadelphia. Travel 3–4 days weekly within an assigned territory is required, with a Monday–Friday schedule and patient-centered focus on ESRD and CKD care.

Responsibilities include coordinating care across teams, optimizing outcomes, and supporting patients, families, and physicians in a dynamic healthcare environment.

Qualifications

• ADN required; BSN preferred; diploma may be substituted for degree
• Current RN license in state of practice required
• Minimum five years of clinical nursing experience
• Three years in renal nursing preferred
• Case management experience required or equivalent patient coordination across teams

Responsibilities

• Coordinate and facilitate care for ESRD/CKD patients across the care team
• Serve as liaison between patient, family, physician, providers and regional operations
• Travel 3–4 days per week within assigned territory and operate hybrid in-home and field-based work
