Job summary
South Texas Renal Care Group is seeking a dedicated Nurse Practitioner to provide comprehensive dialysis care across outpatient units in San Antonio and surrounding communities. You will collaborate with nephrologists and an interdisciplinary team to optimize CKD/ESRD care and ensure high-quality, patient-centered services.

Qualifications
- Current Texas NP license or eligibility.
- Board certification as NP (ANCC or AANP).
- DEA license and prescriptive authority in Texas (or eligibility).
- Minimum 1–2 years clinical experience preferred but not required; dialysis, nephrology, or internal medicine experience strongly preferred but not required.
Responsibilities
- Provide routine and acute clinical care for patients undergoing hemodialysis and/or peritoneal dialysis.
- Conduct initial assessments, follow-up visits, and monthly dialysis rounds.
- Evaluate, diagnose, and manage AKI, CKD, ESRD, and dialysis-related complications.
- Collaborate with supervising nephrologists regarding patient care plans and clinical decision-making.
- Review and act on lab results, dialysis adequacy metrics, and medication regimens.
- Manage comorbid conditions such as hypertension, diabetes, anemia, and fluid/electrolyte imbalances.
- Prescribe medications and adjust treatments in accordance with established protocols and state regulations.
- Participate in care coordination with social workers, dietitians, and dialysis staff.
- Ensure compliance with CMS, ESRD Network, and facility quality standards.
- Educate patients and families regarding disease management, dialysis options and transplant considerations, treatment adherence, diet/fluid management, and lifestyle modifications to slow CKD progression.
- Document patient encounters accurately in the electronic medical record (EMR).
