Job summary

U.S. Renal Care in San Antonio, TX is seeking an Advanced Practitioner to support nephrology services. You will perform ESKD rounding, assess patients, order tests, and coordinate care under practice nephrologists.

The role emphasizes holistic care, patient education, and collaboration with dialysis teams, hospital discharge staff, and PCPs. You should have an active Texas AP license and nephrology experience.

Qualifications

• Current license to practice as an Advanced Practitioner in Texas.
• Firm understanding of protocols, policies and operations of a physician practice.
• 2 years nephrology experience is a strong plus.
• Excellent written and verbal communication skills.

Responsibilities

• ESKD rounding, including limited and comprehensive visits under nephrologists.
• Holistic ESKD patient care management including activation, depression, risk of hospitalization.
• ESKD patient education and annual disease management visits.
• Care collaboration with nephrologists, dialysis teams, hospital discharge staff and PCPs.
• CKD clinical care and kidney disease education for patients.
• Perform examinations, order tests, plan and adjust treatments as needed.
• Document medical information and maintain EMR.
• Promote GUEST customer service standards and team collaboration.
• Attend and participate in team-based care with reliability.
