Job summary

Concentra, Inc. seeks a Physical Therapist Float in Santa Clara to provide rehabilitative services, evaluate motor and neurological disorders, and support coordinated patient care. You will work closely with medical staff to ensure quality services and adherence to policies.

The role emphasizes documentation, patient education, and collaboration with a multidisciplinary team to optimize recovery and return to work for patients in our occupational medicine program.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Master's degree in Physical Therapy preferred.
• Ortho-outpatient experience preferred.
• Ability to maintain patient care records and reports.
• HIPAA and PHI compliance knowledge beneficial.
• Willingness to participate in ongoing training.

Responsibilities

• Deliver high quality patient care and evaluation of motor, skeletal, and neurological conditions.
• Coordinate multidisciplinary treatment plans and documentation.
• Collaborate with physicians to ensure comprehensive patient care.
• Educate patients on care, exercises, and use of supports.
• Ensure proper documentation and compliance with policies.
