Job summary

Concentra is seeking a PRN Physical Therapist in San Jose, CA to provide expert patient care and perform supervisory duties and evaluations. You will help patients improve motor functions, teach tasks for daily living, and collaborate with a multidisciplinary team to ensure high-quality care.

The role emphasizes documentation, HIPAA compliance, and active patient progression through individualized treatment plans within Concentra's occupational medicine framework.

Qualifications

• Bachelor's Degree from an accredited Physical Therapy program.
• Masters' Degree from an accredited Physical Therapy program, preferred.
• Ortho-outpatient experience, preferred.

Responsibilities

• Ensure delivery of high quality patient care services.
• Perform direct patient care, including evaluation of muscle, skeletal, neurological and other physical problems.
• Support and encourage multidisciplinary coordination of treatment.
• Ensure proper documentation of patient care.
• Maintain professional standards through compliance with quality assurance policies/procedures as well as applicable regulations and corporate policies.
• Implement effective communication with staff and patients to ensure coordinated efforts and provision of high quality service and optimal patient care.
• Plan, design, implement and interpret physical therapy treatment programs for patients to facilitate recovery and maximum physical potential.
• Educate patients in proper care and use of supports and exercise programs.
