Job summary

A leading behavioral health organization is seeking an Entitlements Case Manager to coordinate services across multiple provider sites in Baltimore, Maryland. This role offers flexibility as either a W-2 employee or 1099 independent contractor. The ideal candidate will possess a relevant degree, have at least 2 years of case management experience, and strong documentation skills. Competitive salary and benefits are offered.

Qualifications

• Bachelor's degree in Psychology, Social Work, Counseling, Nursing, or related field.
• 2+ years of behavioral health, case management, or health-home experience.
• Strong documentation skills; knowledge of Medicaid and CARF standards.

Responsibilities

• Conduct intake assessments and develop individualized case plans.
• Navigate and maintain eligibility for Medicaid and other support services.
• Coordinate with external providers to address social determinants of health.
• Maintain accurate documentation in the electronic health record.
