A leading behavioral health organization is seeking an Entitlements Case Manager to coordinate services across multiple provider sites in Baltimore, Maryland. This role offers flexibility as either a W-2 employee or 1099 independent contractor. Qualifications
Bachelor's degree in Psychology, Social Work, Counseling, Nursing, or related field.
2+ years of behavioral health, case management, or health-home experience.
Strong documentation skills; knowledge of Medicaid and CARF standards.
Responsibilities
Conduct intake assessments and develop individualized case plans.
Navigate and maintain eligibility for Medicaid and other support services.
Coordinate with external providers to address social determinants of health.
Maintain accurate documentation in the electronic health record.
