Qualifications

• At least one year of experience in case management/coordinating services for individuals with complex medical needs under a Medicaid waiver.
• Supports planning or CCS experience strongly preferred.
• Experience working with medical-based community resources is strongly preferred.

Responsibilities

• Provide ongoing professional case management and advocacy for clients.
• Develop Person-Centered Plans of Service for all clients.
• Conduct care coordination through telephonic outreach and on-site visits.
• Collaborate with community agencies to connect clients to resources.
