Job summary

A healthcare services organization is seeking a Case Manager to coordinate services across multiple sites in Maryland. This role offers W-2 employee or 1099 contractor options, with a salary range of $50,000-$100,000 annually for W-2 and $30-$45 hourly for 1099. Ideal candidates will have a degree in a relevant field, relevant licenses, and experience in behavioral health case management. Qualifications

• 2+ years of behavioral health, case management, or health-home experience.
• Experience with co-occurring or dual-diagnosis populations preferred.
• Ability to work independently and collaboratively.

Responsibilities

• Conduct intake assessments and develop individualized case plans.
• Manage eligibility for Medicaid, SNAP, SSI/SSDI, and other support services.
• Maintain documentation in electronic health records and compliance reports.
