Job summary

Fresenius Medical Care seeks an entry level Social Worker (MSW) to deliver psychosocial services to patients on in-center and home dialysis where applicable. You will work with the healthcare team to promote adjustment, rehabilitation and quality of life for patients and families through education, counseling and care planning.

• Masters in Social Work required or equivalent.
• Must have state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW License (if required by state).
• Obtains necessary state licensure to work independently within the first two years or as state guidelines require.

Responsibilities

• Provide psychosocial assessment, care planning and counseling to patients.
• Assess patients' psychosocial status and collaborate with interdisciplinary team to identify interventions.
• Educate patients and families about treatment options, advance directives, and community resources.
• Document MSW interactions and interventions according to policy.
• Support quality improvement activities and patient satisfaction initiatives.
• Assist with transplant referral processes and waitlist management.
