Job summary

PeerStar is seeking a Social Worker (MSW) to deliver psychosocial assessments, crisis intervention, and peer-support services in a hybrid setting. The role emphasizes recovery-oriented, relationship-based support and connecting residents with community resources.

Strong communication and confidentiality are essential. Ideal candidates hold an MSW with PA LSW preferred but not required, and 1–2 years in geriatric healthcare.

Qualifications

• Master's degree in Social Work (MSW) from a CSWE-accredited program.
• Active LSW licensure in the Commonwealth of Pennsylvania preferred but not required.
• 1–2 years' experience in long-term care, skilled nursing, or geriatric healthcare preferred.
• Working knowledge of OBRA regulations and PA Department of Health nursing home regulations.
• Strong understanding of Medicare/Medicaid psychosocial documentation requirements.
• Willingness to complete PA peer support training if required.
• Excellent interpersonal, communication, and crisis-intervention skills.
• Ability to collaborate with an interdisciplinary team and maintain confidentiality.

Responsibilities

• Serve as a peer support resource for residents, drawing on lived or professional experience.
• Help residents set, revise, and attain personal recovery goals.
• Support residents transitioning into the facility and adjusting to changes.
• Provide outreach to residents who withdraw or need additional engagement.
• Assist residents in voicing needs to staff and participate in care planning.
• Identify barriers to emotional wellbeing and develop strategies to address them.
• Help residents leverage natural supports and community resources.
• Encourage ongoing attendance at appointments, therapies, and activities.
• Provide crisis support and de-escalation in coordination with clinical staff.
• Educate residents, families, and staff about mental health to reduce stigma.
