Job summary

PeerStar is seeking a Social Worker (MSW) to provide psychosocial assessment, crisis intervention, and peer support to residents and families in a hybrid role. The position emphasizes recovery-oriented, relationship-based care within a skilled nursing setting, integrating clinical and peer support.

Requires an MSW and PA licensure considerations, plus 1–2 years in geriatric care; strong interpersonal and crisis-intervention skills are essential, with a commitment to confidentiality and team

Qualifications

• MSW from a CSWE-accredited program is required.
• Active LSW licensure preferred but not required in PA health settings.
• 1–2 years in long-term care, skilled nursing, or geriatric healthcare preferred.
• Knowledge of OBRA and PA health regs; Medicare/Medicaid docs experience.
• Ability to draw on lived/professional experience to connect with residents.

Responsibilities

• Serve as a peer support resource, drawing on relevant experience to help residents.
• Assist residents in setting and attaining recovery- and wellness-oriented goals.
• Support residents transitioning into the facility or adjusting to changes in condition.
• Provide outreach to residents who withdraw or need additional support.
• Help residents advocate for themselves in care planning and daily life.
• Identify barriers to wellbeing and develop strategies to overcome them.
• Promote use of natural supports and community resources.
• Support residents in building social skills and coping strategies.
• Encourage attendance at appointments, therapies, and activities.
• Provide crisis support and de-escalation with clinical staff as needed.
• Educate residents, families, and staff to reduce stigma around mental health.
