Job summary

LiveWell Homecare Agency is seeking a Medical Social Worker (MSW) to provide psychosocial assessments, counseling, and care coordination for home health patients. You will collaborate with the care team to align services with patient goals and healthcare plans.

The role requires strong ethical standards, HIPAA knowledge, and the ability to navigate community resources to support patients and families through transitions and challenges.

Qualifications

• Master’s degree in social work.
• Minimum 1 year of social work or mental health experience.
• Effective communication with diverse populations and adherence to NASW Code of Ethics.
• Knowledge of patient rights, laws (like HIPAA), and psychological principles.
• High emotional intelligence, empathy, and reasoning skills.
• Preferred: LSW license in Pennsylvania.

• Perform initial psychosocial evaluations through patient interviews.
• Report suspected cases of abuse or neglect.
