Job summary

PeerStar is seeking an MSW to provide psychosocial assessment, crisis intervention, and peer support within a hybrid Social Worker role. You will work with residents and their families, helping build self-help skills and connect with community resources.

The position requires an MSW, PA regulatory knowledge, and the ability to collaborate with an interdisciplinary team to promote dignity, recovery, and quality of life in a skilled nursing setting.

Qualifications

• Master's degree in Social Work (MSW) from a CSWE-accredited program.
• Active LSW licensure preferred but not required; PA generally exempts some social workers in health care institutions from licensure.
• Minimum of 1–2 years' experience in a long-term care, skilled nursing, or geriatric healthcare setting preferred.
• Working knowledge of OBRA regulations and PA Department of Health nursing home regulations.
• Strong understanding of Medicare/Medicaid psychosocial documentation requirements.
• Willingness to complete a peer support training curriculum recognized by Pennsylvania if required.

Responsibilities

• Serve as a peer support resource for residents, drawing on personal or professional experience.
• Assist residents in setting, revising, and attaining recovery- and wellness-oriented goals.
• Support residents transitioning into the facility or adjusting to changes in condition.
• Provide outreach and re-engagement to residents who withdraw or are isolated.
• Help residents have their voices heard in care planning and communicate needs to staff.
• Help residents identify barriers to emotional wellbeing and independence and develop strategies.
