Job summary

PeerStar is seeking a Social Worker (MSW) to deliver psychosocial assessment, crisis intervention, and peer-support services for residents and their families in a skilled nursing facility. The role blends clinical social work with recovery-oriented, relationship-based peer support to enhance recovery, wellness, and community connection.

Requirements include an MSW from a CSWE-accredited program and active PA LSW licensure preferred.

Qualifications

• MSW degree from CSWE-accredited program.
• Active LSW licensure preferred but not required in PA, which often exempts some social workers.

Responsibilities

• Provide peer support services drawing on lived/professional experience to support residents.
• Assist residents with setting recovery- and wellness-oriented goals.
• Provide crisis support and coordinate with clinical staff.
• Assist in care planning and voicing resident needs to staff; maintain confidentiality.
