Lutheran SeniorLife is seeking a Social Worker (MSW) in Beaver County, PA, to join the LIFE Program within an Interdisciplinary Care Team. The role focuses on assessing, teaching and counseling to support mental and social health for enrolled participants in their home and community.

Qualifications

• Master’s Degree (MSW) in Social Work from an accredited school of social work.
• Minimum of one year’s experience with a frail or elderly population.
• Working knowledge of computers; Word and Excel required.

Responsibilities

• Promotes and maintains mental and social health of enrolled LIFE participants through assessment, teaching and counseling.
• Facilitates communication between participants, families and Care Team.
• Implements social work care plans and coordinates with other services.
