Lutheran SeniorLife is seeking a Social Worker (MSW) to join an Interdisciplinary Care Team in Beaver County, PA. The role focuses on assessing, teaching and counseling to support LIFE program participants and their families within a home-and-community based care model.

The position requires an MSW with experience with frail or elderly populations, strong communication skills, and proficiency with Microsoft Word and Excel. A sign-on bonus is offered and benefits are provided.

Qualifications

• MSW from an accredited school of social work.
• Experience with frail or elderly population preferred.
• Knowledge of interdisciplinary care management.

Responsibilities

• Promotes and maintains the mental and social health of enrolled LIFE participants.
• Implements social work care plan and coordinates with Care Team.
• Facilitates communication between participants, families and team.
