Lutheran SeniorLife is seeking a Social Worker (MSW) to join an Interdisciplinary Care Team serving LIFE program participants in Beaver County, PA. You will assess and support mental and social health, coordinate with families, and implement care plans in a dedication to community-based services.
The role requires an MSW with at least 1 year of frail elderly experience and comfort with care coordination, MS Word/Excel, and computer usage. Full-time daytime hours with a $4,000 sign-on bonus.

Full-Time, Daylight shift of 8:00 a.m. to 4:30 p.m.

The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.
Social Worker (MSW) is within an Interdisciplinary Care Team setting, incumbent promotes and maintains the mental and social health of enrolled participants through assessment, treatment, teaching and counseling. Provides basic casework and consultation for LIFE Program participants. Facilitates communication between participants, their family and the Care Team. Responsible for the implementation of social work care plan and coordination of social work with other services.
