Job summary

Senior LIFE is seeking a Social Worker to join our home- and community-based Medicare/Medicaid program. The role involves interdisciplinary team collaboration, assessment, and ongoing care planning for LIFE Center members living at home.

Responsibilities

• Oversee direct social work case management for LIFE Center members (caseloads up to 50–60).
• Provide social service support including screenings, counseling, and external referrals as needed.
• Reevaluate and update Plans of Care based on member status and communicate changes to interdisciplinary teams and families.
• Document visits, care plans, and interventions; maintain electronic case files.
• Identify development needs and opportunities for LIFE Center growth and implement improvements.
• Counsel families in individual and group settings and lead education activities for participants.
• Visit in-home members to assess needs and develop speech/language care plans where appropriate.
• Perform general administrative duties and complete required agency documents.

Responsibilities

The Social Worker will be a key participant in member interdisciplinary teams, weighing in on a host of medical, physical, social, and emotional assessments and Plans of Care. Additionally, this professional will oversee a number of core programming and therapeutic responsibilities, including:

• Overseeing direct social work case management services for caseloads of up to 50-60 LIFE Center members
• Providing social service support including screenings, counseling and external referrals, as needed
• Reevaluating and updating Plans of Care based on change in members’ status, emergent conditions and other relevant circumstances, then communicating changes to all relevant stakeholders (i.e. interdisciplinary teams, families, caregivers)
• Completing ongoing periodic assessments to ensure a thorough evolution of member care, in keeping with changing needs and requests
• Documenting all visits, including deviations from approved care plans and prescribed intervention techniques
• Documenting member medical records and maintaining electronic case management assessments for immediate and future use
• Identifying development areas, including individual member enhancements and overarching LIFE Center opportunities for growth and expansion, then developing and implementing these improvements
• Counseling families in both individual and group settings, and leading relevant activities to educate these care participants
• Visiting in-home members, assessing needs and developing/implementing comprehensive speech/language care plans accordingly
• General administrative duties, including completing and submitting 1768 documents to required agencies

Qualifications
