Job summary

Qualifications

• Master's degree in social work required.
• Licensure or intern status as social worker (LCSW / LMSW/RCSW-I) preferred.

Responsibilities

• Perform non-clinical biopsychosocial assessments and interventions for inpatient floors and the ED.
• Provide crisis intervention, patient/family support, and referrals for financial or resource assistance.
• Assist in post-acute placement and discharge planning for high-risk patients.
• Coordinate meetings with care teams and families to support patient-centered decisions.
• Offer emotional support and advocacy for patients and families facing chronic or terminal illness.
