Qualifications

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient needs.
• Develop individualized care plans with the care team.
• Provide emotional support, counseling, and crisis intervention.
• Coordinate with RN Case Managers, physicians, nurses, therapists, and social workers.
• Facilitate referrals to community resources and social services.
• Ensure seamless transitions from hospital to home or long-term care.
• Educate patients and families about post-discharge care and resources.
• Document assessments, care plans, interventions, and outcomes in EPIC.
• Address social determinants of health and connect to resources.
