Qualifications

Responsibilities

• Conduct psychosocial assessments to identify risks, service needs and goals.
• Formulate and update individualized care plans with participants and families.
• Provide individual counseling and facilitate family conferences and support groups.
• Connect participants with community resources and coordinate discharge planning.
• Serve as liaison between participants, family networks, clinical team and external agencies.
• Participate in IDT meetings and advocate for participant needs.
• Complete documentation and ensure HIPAA-compliant records.
