TEEMA Solutions Group is seeking a Social Worker (MSW) for a full-time contract in a hybrid setting in San Francisco, CA. The role focuses on clinical case management for vulnerable older adults, including assessments, care planning, counseling, and resource coordination.

The position requires an MSW, California licensure, and at least one year of eldercare experience, with strong teamwork and EHR proficiency. Hybrid work environment with standard business hours.

Formación

• Master's Degree in Social Work (MSW) from an accredited college or university.
• Valid California Social Worker license/registration.
• Minimum of one year of eldercare experience.

Responsabilidades

• Conduct psychosocial assessments to identify risks, service needs and goals.
• Formulate and update individualized care plans with participants and families.
• Provide individual counseling and facilitate family conferences and support groups.
• Connect participants with community resources and coordinate discharge planning.
• Serve as liaison between participants, family networks, clinical team and external agencies.
• Participate in IDT meetings and advocate for participant needs.
• Complete documentation and ensure HIPAA-compliant records.
