Qualifications

• MSW required from an accredited program.
• CA MSW license required for practice.
• Bachelor’s in Social Work may be considered with experience and language proficiency.
• Proficiency in Microsoft Office and EHR systems.

Responsibilities

• Clinical practice and case management for a panel of patients.
• Conduct psychosocial assessments and develop individualized care plans.
• Coordinate with caregivers and support systems for coordinated care.
• Provide counseling and refer to community resources as needed.
• Engage with community partners and participate in team meetings.
