Job summary

NYU Langone Orthopedic Hospital is seeking a MSW Social Worker to join our team. Under supervision, the successful candidate provides psychosocial assessment and counseling, including discharge planning, to patients assigned by the Department of Social Work/Care Management.

The role involves educating patients and families, coordinating with interdisciplinary teams, and supervising social work interns. Strong communication and compassionate care are essential.

Qualifications

• Masters Degree in social work, or its equivalent, LMSW licensure or NYS permit.
• Required coursework or training in identification and reporting of child abuse (Education Law 6507(3)(a)).

Responsibilities

• Performs other duties as assigned.
• Provides timely and effective psychosocial assessments and evaluations; gathers social history, defines problems and develops treatment plan.
• Coordinates interdisciplinary information into social work treatment plan and establishes post-hospital/discharge plan.
• Implements plan for psychosocial assessments and evaluations with appropriate interventions for patient/family dynamics.
• Develops and conducts group programs as appropriate to assigned service; maintains therapeutic relationship with patient/family.
• Documents all social work activities in a timely manner in HIS/outpatient records, including cultural/spiritual needs.
