Job summary

• Provides discharge planning and education to patients and families.
• Documents all social work activities timely and accurately in HIS/outpatient records.
• Supervises social work interns and participates in interdisciplinary rounds and meetings.
• Establishes post-hospital/discharge plans and coordinates with Discharge Planning Unit.
