Job summary

NYU Langone Health in New York is seeking a Social Worker in Medicine to provide psychosocial assessment and discharge planning for patients under the Department of Social Work. You will collaborate with care managers and interdisciplinary teams to support timely post-hospital care and coordinate community resources.

Qualified candidates will hold a MSW from an accredited program and NYS LMSW/LCSW licensure or permitted status.

Qualifications

• Master's degree in social work or equivalent with licensure eligibility in New York.
• New York State LMSW/LCSW licensure or NYS permit required.
• CSWE accreditation recognized by NYS Education Department.

Responsibilities

• Participates in interdisciplinary planning and unit-based meetings.
• Performs psychosocial assessments and develops treatment plans.
• Coordinates with Care Manager to ensure throughput and timely discharge planning.
• Documents all activities per departmental protocols in medical records.
• Provides supervision to social work interns when qualified (3 years post-graduate).
• Educates patients and families about diagnoses, treatment options, and post-hospital care.
