Job summary

InnovAge is seeking an MSW Social Worker to plan, organize and implement social work services for participants and caregivers. The role is on-site at the center with field visits to participants' homes to ensure continuity of care.

Responsibilities include psychosocial assessments, care planning, IDT collaboration, discharge planning, and transitions. The position emphasizes advocacy, documentation in Epic/EireneRx, and coordination with facilities and community resources.

Qualifications

• Master's degree from an accredited school of social work.
• One year of healthcare setting experience.
• Experience conducting psychosocial assessments, care planning and case management.
• Requires personal transportation and valid driver’s license.
• Californian CPR/BLS and First Aid are required.
• Licensed LMSW or LCSW preferred.

Responsibilities

• Assess psychosocial needs and participate in care planning.
• Serve as liaison between participant, family and the IDT.
• Coordinate admissions, discharges, transportation and transitions.
• Document in Epic and other systems accurately and timely.
• Attend IDT meetings and contribute to care conferences.
• Support end-of-life and hospice-related activities as needed.
• Advocate for safe housing and coordinate with external services.
