Job summary

Adaptive Home Health is seeking a Medical Social Worker (MSW) to conduct psychosocial assessments, connect families with resources, and support patient transitions in a field-based role.

You will collaborate with physicians and the care team, provide counseling, and help navigate social determinants of health. A CSWE-accredited MSW degree and an LMSW/LCSW license are required, with 1+ year in healthcare social work and valid driver's license. Mileage reimbursement offered.

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work.
• Michigan LMSW or LCSW license (or state licensure as required).
• Minimum 1 year of healthcare social work experience (home health, hospice, hospital, or community health preferred).
• CPR certification (BLS).
• Valid driver's license, auto insurance, and reliable transportation.
• Strong knowledge of community resources including housing, financial assistance programs, mental health services, and funeral/legal planning.
• Excellent oral and written communication and interpersonal skills.
• Ability to organize tasks, set priorities, and work autonomously in field-based settings.
• Flexibility with work hours and ability to travel locally.
• Fluency in English (written and spoken).

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs related to illness, recovery, and adjustment to care.
• Evaluate the relationship between medical/nursing needs and available home resources, including financial, community, and caregiver support.
• Connect patients with community resources to address barriers (housing, transportation, financial assistance, medication programs, legal services, funeral planning).
• Provide counseling and education to patients and families on coping with emotional and social challenges.
• Collaborate with physicians and inter-disciplinary team members to communicate psychosocial factors affecting health and adherence.
• Participate in development and evaluation of the physician's Plan of Care to address social and emotional needs.
• Monitor and report changes in patient condition and psychosocial status to the care team.
• Participate in discharge planning to ensure continuity of care.
• Serve as a resource to the team for identifying and resolving psychosocial needs.
• Supervise and evaluate Social Work Assistants where applicable.
