RWJBarnabas Health is seeking a Social Worker MSW to join the Case Management team at RWJ Somerset in Somerville, NJ. You will perform comprehensive assessments, develop discharge plans, and provide counseling to patients and families as part of an interdisciplinary team.

The role requires an MSW and NJ licensure (LSW or LCSW), with a weekend/holiday rotation. Bilingual English/Spanish skills are highly preferred, and hospital experience is advantageous.

Qualifications

Master's degree in Social Work (MSW) from an accredited School of Social Work.
Active Social Work Licensure in the State of New Jersey (LSW or LCSW).
Flexibility to participate in weekend and holiday work rotations.
Bilingual English/Spanish skills are highly preferred.
Prior medical social work or hospital case management experience preferred.

Responsibilities

Identify, assess, and document clinical needs of patients across hospital populations.
Develop and implement discharge plans tailored to each patient.
Provide counseling to patients and families about illness, hospitalization, and care transitions.
Connect patients with community resources and coordinate post-acute care and follow-ups.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

As a Social Worker MSW, a typical day might include the following:

Identifying, screening, and completing comprehensive clinical assessments for patients across assigned hospital populations.
Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient's unique needs.
Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions.
Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups.
Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs.
Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning.
Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities.
Entering accurate, clear, and timely clinical documentation and intervention records into the patient's electronic medical chart.
