Job summary

RWJBarnabas Health's Case Management team at Clara Maass Medical Center is seeking a Social Worker MSW to advocate for patients across the healthcare continuum.

You will perform comprehensive clinical assessments, prepare discharge plans, provide counseling, connect with community resources, and document in the EMR. NJ licensure is required, and weekend/holiday rotation is expected. Bilingual English/Spanish is highly preferred.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited School of Social Work.
• Active Social Work Licensure in the State of New Jersey (LSW or LCSW).
• Flexibility to participate in weekend/holiday work rotation.

Responsibilities

• Identify, screen, and complete comprehensive clinical assessments for patients.
• Formulate and implement safe discharge plans tailored to each patient’s needs.
• Provide supportive counseling to patients and families regarding illness and care transitions.
• Connect patients with community resources, medical equipment, or post-acute care, with diligent follow-ups.
• Participate in interdisciplinary team rounds to review caseloads and advocate for patient needs.
• Coordinate social work services within the palliative care team and facilitate family meetings.
• Serve as a liaison between hospital teams, QA, hospices, home care agencies, and long-term facilities.
• Enter accurate clinical documentation into the patient’s EMR.

As a Social Worker MSW, a typical day might include the following:

• Identifying, screening, and completing comprehensive clinical assessments for patients across assigned hospital populations.
• Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient’s unique needs.
• Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions.
• Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups.
• Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs.
• Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning.
• Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities.
• Entering accurate, clear, and timely clinical documentation and intervention records into the patient’s electronic medical chart.
