Job summary

RWJBarnabas Health is seeking a Social Worker MSW to join the Case Management team at RWJ Somerset in Somerville, NJ. You will perform comprehensive assessments, develop discharge plans, and provide counseling to patients and families as part of an interdisciplinary team.

The role requires an MSW and NJ licensure (LSW or LCSW), with a weekend/holiday rotation. Bilingual English/Spanish skills are highly preferred, and hospital experience is advantageous.

Qualifications

• Master's degree in Social Work (MSW) from an accredited School of Social Work.
• Active Social Work Licensure in the State of New Jersey (LSW or LCSW).
• Flexibility to participate in weekend and holiday work rotations.
• Bilingual English/Spanish skills are highly preferred.
• Prior medical social work or hospital case management experience preferred.

Responsibilities

• Identify, assess, and document clinical needs of patients across hospital populations.
• Develop and implement discharge plans tailored to each patient.
• Provide counseling to patients and families about illness, hospitalization, and care transitions.
• Connect patients with community resources and coordinate post-acute care and follow-ups.
• Collaborate with interdisciplinary teams to advocate for patient needs and ensure coordinated care.
