Qualifications

• RN or Social Worker licensure in NJ is required.
• EARC certification within 6 months of hire.
• Minimum 2 years of acute care case management experience.

Responsibilities

• Provide direct case management services, including assessment, care coordination, discharge planning, and interdisciplinary collaboration.
• Mentor and precept case management staff to support clinical decision-making.
• Coordinate daily workflow, prioritize cases, and optimize throughput.
