Job summary

Piedmont Atlanta Hospital seeks an experienced social worker to promote patient and family wellness and to improve care outcomes through comprehensive transitional planning in inpatient and ED settings. The role requires a Master’s in Social Work and at least 1 year of acute or post-acute experience.

Licensure as LMSW/ACM/CCM is preferred, with a collaborative, multidisciplinary team approach in a large hospital environment.

Qualifications

• Master's Degree in Social Work from an accredited program (Required)
• 1 year of experience in an acute or post-acute setting (Required)
• LMSW, ACM or CCM preferred (optional)

Responsibilities

• Promote patient and family wellness and improve care outcomes.
• Develop transitional care plans for patients with complex and psychosocial needs in Inpatient and ED environments.
• Collaborate with service team members, physicians, patients, external agencies and families to ensure quality care and effective transitions.
