Job summary

AdventHealth is seeking a skilled case manager to collaborate on patient discharge plans and optimize capacity management across the campus. You will work with the Acute Care Management team, patients and families to develop realistic discharge plans while identifying barriers and coordinating resources for a smooth transition.

The role emphasizes interdisciplinary collaboration, patient advocacy, and proactive problem-solving to support quality care and efficient patient flow.

Qualifications

• Master's degree required.
• 2+ acute care experience required.
• 2+ experience in healthcare related fields required.
• Licensure: LCSW, ACM or CCM required.

Responsibilities

• Collaborate and facilitate patient discharge plans initiated by the Acute Care Management team.
• Support capacity management goals by achieving length of stay targets and identifying avoidable delays.
• Collaborate with patient, family and care teams to establish a realistic discharge plan.
• Act as patient advocate by coordinating resources with payers, agencies and vendors during transitions.
