Abrazo West Campus Hospital is seeking a Social Worker to facilitate care coordination across the patient care continuum, focusing on transition planning and discharge needs. The role emphasizes psycho-social assessment, resource identification and patient self-determination.

You will lead a team in transition management, ensure efficient care coordination, and provide education to clinicians and families while adhering to state and federal requirements.

Qualifications

• Master's of Social Work is required.
• Arizona licensed social worker (LMSW) must be obtained within 90 days of hire.
• MIDAS and Curaspan training must be completed within first 90 days.

Responsabilités

• Conduct psycho-social transition planning and reassessment.
• Coordinate care and transition plans to optimize throughput and discharge.
• Lead multidisciplinary patient care conferences, including Complex Case Review.
• Provide education to physicians, patients, families and caregivers.
• Ensure compliance with regulatory requirements and Tenet policies.
