Qualifications

• Masters' degree in Social Work (MSW) is required.
• Licensure must be TX-eligible (LCSW, LMSW, or LMSW-AP).
• 1 year of social work experience preferred.

Responsibilities

• Conducts and documents psycho-social assessments interpreting patient needs.
• Develops and evaluates treatment plans; educates patients and families.
• Provides or supervises care plans including crisis intervention and discharge planning.
• Acts as liaison with community resources and staff for smooth transitions.
• Collaborates with healthcare team to achieve positive outcomes.
• Participates in performance improvement and data collection initiatives.
• Prepares professional reports and case summaries; interprets needs.
