Responsabilidades

• Conduct psycho-social assessments to interpret patient needs.
• Develop, implement, and evaluate treatment plans with the care team.
• Provide or supervise care plans including crisis intervention, bereavement, advocacy, and education.
• Coordinate discharge planning and referrals to community resources.
• Collaborate with the healthcare team to achieve positive patient outcomes.
• Prepare and document professional reports and case summaries.
