Job summary

Qualifications

• Master’s degree in Social Work required.
• TX licensure as LCSW/LMSW/LMSW-AP must be obtained.
• Minimum 1 year of social work experience required.

Responsibilities

• Conduct psycho-social assessments to interpret patient needs.
• Develop, implement, and evaluate treatment plans; educate patients and families.
• Provide or supervise care plans including crisis intervention and discharge planning.
• Coordinate with community resources and staff for smooth transitions.
• Participate in performance improvement and documentation of assessments.
