Baylor Scott & White Health in Waco, TX, is seeking a Social Worker MSW 1 who educates and advises patients and families on coping with emotional or physical issues, in the Emergency Department setting. The role conducts psycho-social assessments, develops care plans, coordinates crisis intervention and discharge planning, and collaborates with the healthcare team to improve patient outcomes.

• Masters degree in Social Work is required.
• TX licensure: LCSW, LMSW, or LMSW-AP required.
• Minimum 1 year of social work experience.
• Experience in Emergency Department case management preferred.

Responsabilités

• Conduct psycho-social assessments and document findings.
• Develop, implement, and evaluate treatment plans with patient/family input.
• Provide or supervise care plans including crisis intervention, bereavement, referrals, discharge planning, education.
• Coordinate with healthcare team to ensure smooth transition and placement.
• Prepare professional reports and case summaries; participate in performance improvement initiatives.
