Baylor Scott & White Health in Waco, TX is seeking a PRN Social Worker (MSW) to educate and assist patients and families as they cope with emotional and medical needs. This role supports crisis intervention, bereavement, referrals, discharge planning, and psychosocial assessments.

The position requires a TX social work license (LCSW/LMSW/LMSW-AP) and at least 1 year of experience, with duties spanning coordination with the healthcare team and community resources.

Formación

• Masters in Social Work required.
• Licensure as TX LCSW/LMSW/LMSW-AP required (one of these).
• Minimum 1 year of social work experience.

Responsabilidades

• Conducts psycho-social assessments to interpret patient and family needs.
• Develops and evaluates treatment plans; provides education to patient and family.
• Creates and supervises care plans; coordinates resources and referrals.
• Liaises with community resources and hospital staff for smooth transitions.
• Collaborates with healthcare teams to achieve positive patient outcomes.
• Participates in performance improvement initiatives and data collection.
