Job summary

Atrium Health is seeking a dedicated social worker to join the Case Management team at Carolinas Medical Center. You will perform psychosocial assessments, assist patients with coping strategies, and collaborate with the Clinical Care Management team to determine the appropriate care setting across the continuum.

Applicants should have a Master’s in Social Work, with LCSW preferred, and relevant experience in hospital or health settings.

Qualifications

• MBA? (not applicable)
• Master's in Social Work required; LCSW preferred.
• Applicable state certification preferred.

Responsibilities

• Completes psychosocial assessments for post hospital care for designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Works with Clinical Care Management to assess and determine appropriate level of care across the continuum.
• Serves as a resource to staff and physicians on emotional and social components of illness and social support.
• Provides information and education to patients on community resources and post-hospital care options; makes referrals as needed.
• Reports suspected abuse/neglect/exploitation; liaises with Department of Social Services during evaluation.
• Responds to after-hours consult requests for patients.
