Job summary

Health First in Florida seeks a Complex Social Worker (MSW) to oversee discharge processes for hospitalized patients with complex barriers to timely discharge. You will coordinate care from admission to post-hospital care and expediting discharges for patients with long lengths of stay.

Responsibilities include developing individualized discharge plans, collaborating with the healthcare team, and guiding families through post-acute arrangements.

Qualifications

• Master’s degree in Social Work (MSW).
• Three (3) years’ experience in medical social work within the last five (5) years.
• AHA BLS Healthcare Provider Completion Card required prior to start date and maintained.

Responsibilities

• Develops and coordinates an individualized discharge plan with post-acute care services.
• Interviews patients and family to assess health, functional, mental, and social support needs.
• Creates a clinically appropriate discharge plan aligned with patient goals and resources.
• Identifies and develops alternatives to overcome discharge barriers.
• Monitors progress and revises plans to minimize discharge delays and communicates plan to the team.
