Job summary

Qualifications

• Master’s degree in Social Work is required.
• Minimum of one year of social work experience or two-semester internship in MWHC Care Coordination.
• Experience with computer technology, Windows-based programs, email, and MS Word is required.
• Healthcare field experience is preferred.
• Case Management Certification is strongly desired.

Responsibilities

• Coordinate care with clinical partners to advance patient outcomes.
• Serve as a subject matter expert for the care team.
• Mentor new social work associates and interns in Care Coordination.
• Assess future patient care needs and independence levels.
• Communicate treatment plan updates with partners and families.
• Develop and communicate discharge plans for medically complex cases.
