Qualifications

  • Master’s degree in Social Work from CSWE-accredited program.
  • At least 1–3 years of Social Work experience in a health care setting.
  • Strong communication and listening abilities.
  • Knowledge of grief, bereavement, and psychosocial assessment.

Responsibilities

  • Assess clients for financial and social needs and determine services required.
  • Interview individuals, families, or groups to discuss needs and plan care.
  • Communicate with management and interdisciplinary team about concerns.
  • Provide bereavement counseling and lead support sessions.
  • Refer clients to community resources for services like counseling or debt guidance.
  • Complete psychosocial assessments, care plans, and discharge plans for hospice patients.
