Job summary

Crossroads Hospice & Palliative Care is seeking a Social Worker (LSW) to support patients and families with emotional and social care within our hospice program.

The role involves collaborating with the care team, participating in interdisciplinary meetings, and coordinating grief support services. A CSWE-accredited MSW/BSW is required, with 2 years in a medical setting preferred. Travel and a valid driver’s license are required.

Qualifications

  • Licensed Social Worker with CSWE-accredited degree.
  • MSW or BSW from CSWE-accredited program.
  • 2 years of experience in a medical setting preferred.
  • Ability to work in hospice & palliative programs.
  • Provide emotional, social, and grief support to patients and families.
  • Participate in Interdisciplinary Group meetings.
  • Assist in coordination of Gift of a Day and Veteran Recognition programs.
  • Reliable vehicle, valid driver’s license, and ability to visit patients.

Responsibilities

  • Provide emotional, social, and grief support to patients and families.
  • Guide families through complex healthcare processes and decisions.
  • Participate in Interdisciplinary Group meetings and address social-psychological issues.
  • Coordinate with spiritual and bereavement staff as part of a care team.
  • Visit patients using a personal vehicle as required.
