MJHS is seeking a dedicated hospice social worker to provide psychosocial support to patients and families across inpatient, home care, and nursing home settings. The role includes participation in an IDG and collaboration with an interdisciplinary team to develop care plans and discharge plans as needed.

The position requires MSW licensure, NYS driver’s license, and at least one year of healthcare social work experience, with proficiency in Word/Excel and data input.

Formación

• Graduate from an accredited School of social work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Driver's license and car required.
• Ability to work within an interdisciplinary team environment.
• Ability to input clinical data proficiently.

Responsabilidades

• You will provide psychosocial support and other Social Work services to Hospice patients and their families in the inpatient, home care/nursing home settings, in pre and post bereavement phases of Hospice service
• You will serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff
• You will develop social work plan of care, goals and makes recommendations based on patient/family needs for assigned patients who are direct admission to inpatient units or nursing home/hospice residents.
• You will develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.
