Job summary

MJHS seeks a licensed MSW to provide psychosocial support to Hospice patients and families across inpatient and home care settings, with emphasis on bereavement care. You will collaborate with an interdisciplinary team and help plan patient-centered goals.

Requirements include NYS MSW licensure, one year in healthcare social work, and proficiency with Word and Excel. A valid NY driver’s license and reliable car are expected for community visits.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Driver's license and car required.

Responsibilities

• Provide psychosocial support and other Social Work services to Hospice patients and their families in inpatient, home care/nursing home settings, in pre and post bereavement phases of Hospice service.
• Serve as social work IDG member and liaison between hospice staff and inpatient and nursing home staff.
• Develop social work plan of care, goals and recommendations based on patient/family needs for assigned patients who are direct admission to inpatient units or nursing home/hospice residents.
• Develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.
