Qualifications

• Master’s in Social Work required.
• 1–2 years post-graduate social work experience in health care.
• Licensed Medical Social Worker in the relevant state.
• Valid driver’s license with ~20% travel to patient locations.
• Drug screening required prior to employment.
• Eligible to work in the United States.

Responsibilities

• Deliver compassionate, patient-centered hospice care.
• Collaborate with an interdisciplinary care team.
• Conduct psychosocial and bereavement assessments.
• Identify patient and family needs and develop supportive care plans.
• Provide counseling and emotional support to patients and families.
• Connect patients with resources and help navigate end-of-life care.
• Advocate for patient wishes in complex situations.
• Ensure accurate EMR documentation in a timely and compliant manner.
• Work independently while collaborating with the care team.
• Travel to patient homes within the assigned territory.
