Job summary

MJHS in the Greater New York area seeks a licensed MSW to provide psychosocial support to hospice patients and families across inpatient, home care, and nursing home settings, including pre and post bereavement.

You will collaborate with an interdisciplinary team, develop plans of care, and coordinate discharge planning. Requirements include NYS MSW licensure, one year healthcare social work experience, and NYS driver's license.

Qualifications

• Graduate from an accredited School of social work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Driver's license and car required.
• Ability to work within an interdisciplinary team environment.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and other Social Work services to Hospice patients and their families in inpatient, home care/nursing home settings, and during pre- and post-bereavement phases.
• Serve as social work member of hospice IDG and liaison between hospice staff and inpatient and nursing home staff.
• Develop social work plan of care, goals and recommendations based on patient/family needs for assigned patients.
• Develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.
