PACE Southeast Michigan is hiring a Master’s Social Worker (MSW) to assess, coordinate, and deliver social services for participants and families in Westland, MI. You will work within the IDT, conducting bio-psychosocial assessments and developing care plans with CBT-informed interventions.

You will coordinate housing, financial, safety, and care needs, advocate for participants, and document care plans in the EHR.

Qualifications

• MSW from an accredited school of social work.
• Licensed to practice in Michigan.
• One year experience as a clinical social worker, preferably geriatrics.
• Knowledge of case management and EHR documentation.

Responsabilités

• Conduct comprehensive bio-psychosocial assessments at enrollment and at 180-day intervals.
• Develop and implement individualized treatment plans with counseling and coordination of services.
• Coordinate housing, financial, safety, and care needs with the care team.
• Assist in discharge planning and educate participants and caregivers.
• Document in the EHR and communicate changes with the IDT.
